I give Paws Adventure permission to have my dog posted on social media via photos or videos from the service/s they attend.

I hereby acknowledge this release from liability for accidental injury or illness which my dog/s may incur as a result of participating in any transport, physical activity and/or play. I hereby assume all risks connected with and consent to participate in this program. I agree to disclose any health or behaviour issues that may affect my dog/s my ability to participate in this program. I acknowledge If my dog injures a member of the public or their dog, I take liability for the occurred incident. Injury to dogs of the public that require veterinary treatment will be covered by me.

I accept cancellation fee of $30 if Paws Adventure is not notified within 24 hours or if unaccessable to take dog/s out for pre-booked session.
